The application behind a military move. DD Form 1299 is the Department of Defense Application for Shipment and/or Storage of Personal Property. It turns orders into a booked shipment: who is moving, from where to where, what it should weigh, and who signs for it. It moved inside the system. Counseling happens in the Defense Personal Property System now, and the 1299 comes out of it already populated. The rest is unchanged: it must be signed, dated and sent to the personal property office with the orders behind it. Until then nothing is booked. What it is ## One application, two jobs. Shipment and storage. The Privacy Act statement on the back names three uses: evaluating the request, preparing the Government bill of lading and other shipping documents, and letting the finance office collect where a shipment exceeds entitlement limits. Authority: 37 USC 406 and 5 USC 5726. One line matters most on the carrier side: the completed form may be given to transportation service providers and their agents **as the official shipping and storage order**. It is the instruction a carrier works from, not a request it interprets. ## Where the paper still bites Military OneSource says the form is generated automatically once counseling is complete, alongside DD Form 1797, the counseling checklist. The signed copy still decides the outcome: Navy guidance says an application is not processed until the orders and both signed forms are uploaded, and DD Form 3160 says the same for non-temporary storage releases. First-time movers, separating members and retirees are told to contact their transportation office first, and practice varies by service and by office. Signatures ## One person signs it. And it is not the counselor. ### The offices own the top Blocks 3, 4 and 5 name the preparing office and the two shipping offices. On a generated form they arrive filled. ### The customer owns the middle Blocks 6 to 16 belong to the customer, which is the word the current edition uses. They are what a counselor checks. ### Block 17 is the signature 17a is the signature of the customer, 17b the date signed. It certifies that the conditions on the back were read. Both halves are required. ### The counselor signs the other form There is no counselor signature block on the DD 1299. That line sits on DD Form 1797. The only other signature here is block 18, a certificate in lieu of signature, used where regulations authorise it. What it captures ## Eighteen blocks. Most of them are yours. ### Who, and which orders Block 6: name, DoD ID, rank or grade, agency, two email addresses. Block 8: permanent or temporary orders, who issued them, the new duty assignment and number. ### Four weights, side by side Block 7 wants the authorized weight, the estimated weight, and estimated professional books, papers and equipment for member and military spouse. ### Who meets the truck Each end gets an address, phone numbers and a named agent to release or receive the property. Block 14 sets earliest, latest and desired pickup. ### The agreement on the back Block 17 signs for the reverse: notify the shipping office if the orders change, consent to collection from pay for excess costs, accept that the government is not responsible for goods left in storage past the authorised period. It cites Chapter 5 of the Joint Travel Regulations. What goes wrong ## The reasons it comes back. Nearly all process, not typing. ### Signed but not dated Block 17 has two halves. A signature over an empty date field is an incomplete form, and an incomplete form is not an application. ### It arrives without the orders Navy guidance lists three uploads before an application is processed: the orders, the signed 1299, the signed 1797. Two of three is none. ### Uploaded, then assumed booked A counselor reviews the application before a shipment goes to a carrier, and official instructions tell the customer to contact the office after submitting. ### Treated as ordinary paperwork The current edition is marked as controlled unclassified information once filled in. A completed form carries a name, a DoD ID, a rank and home addresses at both ends, so it does not belong in a forwarded email thread. It is an Adobe form, so a browser preview may show a placeholder rather than the form. Check the edition line at the foot of the page. The version published there reads **DD FORM 1299, MAR 2022**, stamped as updated in December 2024, beside the words previous edition is obsolete. A DD-1299 off a form site is often the far older edition, which asks for a social security number where the current one asks for a DoD ID. Less than you would think. If you are the person moving, you can stop here. For the company that receives it, the 1299 is not paperwork to complete. It is the shipping and storage order, arriving as instructions.
